If, on the other hand, on injecting saline there is no back-flow, it usually indicates that the needle is lying extra-articularly.

Using the bone block extra-articularly allows the capsular repair anterior to the graft to act as a soft tissue interposition.

The post suture should be free and the lead suture should be in a vertical mattress through the labrum and the graft so that when this suture is tied, appropriate tension is achieved, the graft and the labrum are lying opposed with the graft and any remaining lenth situated extra-articularly (Fig. 7).
